History does repeat itself

How does a society shift its basic assumptions about how things work? Defying Providence tells a fascinating story about one of the most profound changes in our understanding of disease, and how those changes shaped the science, business, and ethics of medicine along the way. Clear writing and impressive research support this dramatic story of how we came to accept a basic medical practice - vaccination - and illuminate the point in our history where evidence-based science began to overtake received wisdom about the workings of God in the natural world.
It is difficult to categorize this book - and that's a good thing. As common as smallpox was, the disease itself and efforts to avoid it touched every aspect of society. In describing the efforts to revise the common understanding and treatment of smallpox, the author touches on epidemiology, the history of medical practice, politics in colonial Boston and Britain, British social history, urban studies and eighteenth century religious philosophies.
All of these disciplines are necessary to illustrate the central theme of Defying Providence - the diffusion of innovation, or how revolutionary ideas make their way into common practice. Most interesting here are the patterns of resistance and adoption. `Upstart' doctors, entrepreneurs, and radical women all play significant roles in the acceptance of inoculation, while resistance tends to come from members of the establishment - whether medical, religious or political - with something to lose. This pattern is one of those that repeats itself if we don't pay attention - consider American debates over global climate change - and this particular iteration makes for an informative and thought-provoking book.

Arthur Boylston has researched and assembled the best account available of how scientific medicine began in 18th century Britain with the evaluation of inoculation (variolation) against smallpox. This is a fascinating and overlooked story, without which there must be doubt that vaccination would have been taken seriously and used in the 20th century to eradicate smallpox. In addition to introducing readers to unsung heroes in this story - Nettleton, Jurin, Watson, Sutton, and others - Boylston gives Edward Jenner credit for what he achieved, while exposing as baseless some hagiographical accounts of the man. Boylston's new book is not only a great contribution to the history of medicine, but a great read!
